Athlete Intake Form

After you have signed up for a monthly training subscription, please complete the athlete intake form. This only has to be completed once. It takes about 20-30 minutes to complete. You can save your progress if you need to come back to it.

Basic Abilities Assessment

Read each statement and decide if you agree or disagree as it applies to you. Check the appropriate answer. If unsure, go with your initial feeling. For triathletes, do this three times – once for swimming, biking, and running. Otherwise, submit a form for only the sports you’re being coached in. Then, schedule a time to review and discuss.

Post Season Review & Survey

Annual athlete review, planning tool, and survey. Please submit after your final race of the season. Then, schedule a time to review and discuss.
